Fantastic Management/Ownership Opportunity for a CHT to Join a Well Established and Highly Regarded Physical Therapy Practice in NYC/LI

Practice Leader- For a New Hand Therapy Division of Evolve Physical Therapy with a Path to Earned Equity
Location: Syosett, Long Island and NYC
Company: Evolve Physical Therapy and Sports Rehabilitation

Evolve Physical Therapy and Sports Rehabilitation is seeking a hands-on (no pun intended) operational leader who wants to build and grow a hand therapy division in the company. The practice will be moving into a new facility on Long Island, building out their adult fitness, youth strength and conditioning as well as baseball and softball training.

This is the optimal time for Evolve to branch into a hand therapy service, building on over 20 years of experience working with a variety of orthopedic patients, deep rooted connections with many orthopedists in the Tri-State area and a long affiliation with HSS and Mt Sinai. The company feels strongly that having on site hand therapy services in their Long Island  and Upper East Side office will close any gaps in the continuum of services and become an integral part of what the company offers.  This position will primarily be in the Long Island office to start.

The ideal candidate should have

  • Current New York State licensure as an Occupational Therapist (OT) or Physical Therapist (PT) with Certified Hand Therapist (CHT) credential required
  • Minimum of 3-5 years of clinical experience in hand and upper extremity rehabilitation
  • Demonstrated leadership experience, whether managing a team, mentoring clinicians, or running a practice
  • Strong relationships with hand surgeons, orthopedic specialists, or referral networks in the Tri-State area (preferred)
  • Entrepreneurial mindset with a passion for building something from the ground up

Responsibilities:

  • Establish and grow Evolve’s hand therapy division across three office locations
  • Maintain a clinical caseload while developing operational infrastructure, protocols, and standards of care
  • Recruit, train, and mentor hand therapy staff as the division expands
  • Cultivate and strengthen referral relationships with orthopedic surgeons and hand specialists
  • Collaborate with leadership on business development strategy, marketing, and community outreach
  • Ensure exceptional patient outcomes and experience consistent with Evolve’s reputation

What We Offer:

  • Competitive base salary commensurate with experience
  • Clear path to earned equity ownership in the hand therapy division
  • Opportunity to shape and lead a new service line within an established, respected practice
  • Collaborative, supportive culture with a 20+ year track record of clinical excellence
  • Modern facilities and resources to support your vision
  • Benefits package including health insurance, continuing education support, and PTO
  • Opportunity for Earned Equity/Partnership
