PT Superbill

No need to reinvent the wheel. Here is a template for an PT super bill which is editable and downloadable so that you can include your practice name etc. You can also customize to include the CPT codes and ICD 10 codes you commonly use in your practice. This can be given to patients as a receipt, and it includes a statement that you make no representation that they will be reimbursed in part or in full if they submit this to their insurance company, FSA or HSA.  Please note that many carriers now require you to put the modifier GP next to all CPT codes to indicate physical therapy. You can add that to the superbill as needed.
